Family of Pharmacies, The Little Clinic LLC, and their dietitians (“Kroger Health”) your phone number, you are consenting to receive the following types of notifications, that may be delivered using pre-recorded or other automated messages. Where required by law, express written consent or authorization will be obtained separately before sending certain types of communications. The notifications that may require express written consent are marked with an asterisk (*) and are summarized below. Consent or authorization can be revoked at any time and is not a condition of purchasing any goods or services. Message and data rates may apply. Message frequency may vary.

You agree that these messages may include limited personal information about you, and whoever has access to the mobile phone or carrier account will also be able to see this information. Kroger Health Pharmacy Notifications include voice or text notifications related to pharmacy services including, but not limited to, prescription pick-up reminders, prescription received notifications, prescription progress notifications, prescription ready notifications, Auto Refill notifications, or other refill management program notifications, and Pharmacy appointment reminders.
